Whole Health Solutions uses the latest Electronic Medical Record technology to communicate with you securely and confidentially via email. We will request your email address so that we can give you access to our completely private, individualized patient portal. Not only do we promise we will never give your email address to anyone else, we must follow HIPAA regulations to keep your information completely confidential.

You can use this secure patient portal for the following:

        1.) Access all patient demographic and health history forms securely online 24 hours per day, 7 days per week. By sending us your visit information anytime before your visit,          we can help speed you through visit check-in, and keep your visit on time.

        2.) Request an appointment.

        3.) Request medication refills.

        4.) Ask clarifying questions when the doctor is unavailable.

        5.) Receive your medical records electronically via secure email

        6.) Have your medical records transferred electronically to yourself or your other doctors. You must give us a signed release prior to this being completed.

        7.) Order nutritional supplements to be delivered to your home. Please see our Products link for details.


NEW PATIENTS: All new patients will receive an email notification into the email box you gave us at the time you arranged your appointment. Please open this message and follow the instructions so that we can perform all necessary procedures on your first visit.  The message will say “Important message from Whole Health Solutions” in the subject line. This will give you the secure information to open your own patient portal. Make sure to record your password and verification code so that you can use the patient portal in the future.

ALL NEW PATIENTS: Please fill out PATIENT  REGISTRATION FORM, LIFESTYLE MEDICINE HEALTH  HISTORY, AND HEALTH PROFILE QUESTIONNAIRE (HPQ) prior to your visit.
